First Visit
New Patient Information
What to Bring to Your First Visit
- A valid photo ID
- A list of current medications and supplements
- Any relevant imaging: MRI, X-ray, or ultrasound reports
- Prior treatment records or referral notes
- A completed new patient intake form
What to Expect at Your First Appointment
Your first visit is a comprehensive consultation. Dr. Chabla will review your history, discuss your symptoms and goals, examine the affected area, and review any prior imaging. He will then walk you through your options and, where appropriate, develop a treatment plan at the same appointment. Plan for approximately 60-90 minutes for your first visit.
PREPARATION
Preparing for Common Procedures
Before a PRP Injection
Avoid NSAIDs (ibuprofen, naproxen, aspirin) for at least 14 days prior. Stay well hydrated in the 24 hours before your appointment. Eat a light meal before your visit. Wear comfortable, loose-fitting clothing that allows access to the treatment area.
Before a Ketamine Injection
Do not eat or drink for at least 8 hours prior to your infusion. Arrange for a responsible adult to drive you home - you may not drive after a ketamine infusion. Avoid alcohol and recreational substances for 24 hours prior. Bring a list of all current medications.
Before Pain Injection Therapies
No special preparation is typically required. Wear comfortable clothing that allows easy access to the treatment site. Let us know if you are on blood thinners or have a pacemaker.
AFTERCARE
After Your Procedure
Mild soreness or swelling at the treatment site is normal and typically resolves within 48-72 hours. Avoid strenuous activity for 24-48 hours unless instructed otherwise. Follow any specific aftercare instructions provided by Dr. Chabla at your appointment. Contact us if you experience severe pain, fever, redness, or swelling at the injection site.

FAQ
Frequently Asked Questions
Do I need a referral?
No, a referral is typically not required to schedule an appointment with Dr. Chabla, as we are a private, direct physician-led practice. However, we always welcome referral notes and prior clinical data from your primary care doctor or physical therapist.
Do you accept insurance?
Chabla Pain & Performance is a private cash-pay clinic. We do not participate in or bill insurance panels directly. This model allows us to prioritize high-touch, outcome-focused clinical care without arbitrary insurance limitations. We gladly provide diagnostic superbills for potential out-of-network reimbursement.
How many treatments will I need?
This depends entirely on your diagnosis and tissue response. Many acute injuries show notable functional improvement within 1-2 PRP or shockwave sessions. For chronic central sensitization or complex neuropathic pain, standard clinical protocols may require a structured series of 3 to 6 sessions.
Are your treatments FDA-approved?
All procedures, biologics, and clinical protocols utilized by Dr. Chabla (such as advanced nerve blocks and ketamine protocols) are evidence-based, medically safe, and supported by extensive peer-reviewed literature.
How soon can I return to activity after treatment?
Diagnostic blocks and shockwave therapy typically carry immediate return-to-activity. Regenerative biologics (like PRP) trigger a localized healing response that may require 24-48 hours of relative rest before starting structured physical rehab.
Is Dr. Chabla the only provider?
Yes. To maintain exceptional clinical outcomes and continuity of care, Dr. Chabla personally conducts every single diagnostic evaluation, designs each treatment plan, and performs all interventional procedures himself.
